1. Auxano is currently accepting new patients, ages 18 and older, for medication management. We are not accepting new therapy patients at this time.


2. Please review our Fees and Insurance page, and call your insurance company to verify that we participate with your insurance plan.


3. After verifying your insurance, please call us at 518.980.9810 and leave a message at extension 2. Someone will call you back to perform a brief intake and determine whether we are a good fit for your needs. If so, they will also schedule your first appointment.


4. Once you have an appointment, please complete your new patient documentation on the Patient Portal. You will use the username and password that you were given when you scheduled your appointment. It is essential that this documentation is completed prior to your appointment. If you are not able to complete your documentation, please call us at 518.980.9810 and leave a message at extension 2.


5. Your first visit is scheduled in the office. Please visit Contact and Directions for directions to our office.


6. If you wish to schedule telehealth visits after your first appointment, please discuss this with your provider.
